This form must be completed for a “Booking Not Required” activity, including Joint Club Wednesday Walks (JCWW). All Participants, whether they are CBC members or not, must sign it for insurance and proper record keeping. (Being a member/guest on Bilby is not sufficient as that does not record who attended an activity.) Anybody who turns up to a “Booking Required” activity but did not book on Bilby, i.e. “Ring-ins”, must also sign the form. Leaders should therefore take a copy of this form to all CBC activities they lead.

After the activity, Leaders must enter the names of any CBC Members/Guests on the form (who did not book on Bilby) onto the Participant list on Bilby (except for JCWW). If there is anybody left on the form who is not a CBC member/guest then the leader must send a scanned copy of the completed form to the Check-In Officer at check.in”at”canberrabushwalkingclub.org (replace “at” with @). For JCWW, just scan the form and send it regardless. The Leader must also click the “Finish Activity” link in Bilby so the activity is marked as Completed. Use this form for all activities involving temporary members (guests) under the age of 18.

Incident report form (PDF) – Reviewed and revised May 2020

Use this form to report:

  • an injury requiring, or likely to require, medical attention, or
  • a significant incident.

A significant incident is:

  • one in which there was a need to involve outside authorities,
  • an unanticipated and serious threat to the safety of the party,
  • a lengthy and unexpected delay to the progress of the party,
  • major damage to property, or
  • any other unforeseen event about which the next group walking in the area should be forewarned.
